Building Use Request Form

This form is to be used by anyone requesting use of the facilities of Laketon Bethel Reformed church for an event other than a wedding reception or funeral.    

• Parties are responsible for all table and place settings: tablecloths, napkins, plates, plastic ware etc.
• No alcohol is allowed on the church premises.
• No fund-raising events are allowed on church property.
• All activities/parties on Saturday must be completed by 4:00pm in order to allow time for preparation for Sunday Worship.
• You will be responsible for cleaning up after your event.  This will include taking out trash, vacuuming, washing down tables and countertops, doing dishes, mopping kitchen floor, etc.  You will be shown where the cleaning supplies are kept.  Please leave the tables and chairs as you find them.
• Any charges/fees for use of the facility for an event are to be predetermined.

When entering times below, please include time needed for set-up and clean-up.

Requests will be reviewed on Tuesdays and Thursdays.  You will be contacted as soon as a decision has been made.  Once you have been approved, if any changes or cancellations are necessary please notify the office promptly at 231.744.1749. 

Funerals and weddings will take precedence over all other events.
